Repeat Prescriptions
You may be able to request a repeat prescription without attending the doctor. If however the Doctor needs to see you then we will contact you in due course.
There is an administration charge of €12 to issue a prescription unless you are a Medical Card holder.
To avoid errors we regret that prescriptions cannot be ordered over the telephone. Please leave in the completed form at least 1 day before your prescription is due for renewal.
You will need to complete a “Prescription Renewal Form” which is available at reception or
please click here and print.
Once you have completed the form you can…
1. Drop/post it to reception of your clinic
2. Fax or email it to the relevant clinic
The prescription will be available for collection from your clinic from 4pm of the following working day. Please make allowances for weekends and public holidays. The €12 administration charge is payable at reception upon collection and a receipt will be issued to you also.
Please inform us if someone other than the patient will be collecting the prescription. Alternatively we are happy to post your prescription to you if you supply us with a stamped addressed envelope.
PRACTICE PRESCRIBING POLICY
In The Meridian Clinic we want to provide you with the best quality medical care. This includes the way medicines are prescribed. For example you might notice how we try to avoid unnecessary antibiotics, for example in treating the 'common cold'(see HSE for further information).
